    What Is a DBQ (Disability Benefits Questionnaire)?

    If a nexus letter is used to prove why you have a condition, a Disability Benefits Questionnaire (DBQ) is used to prove how severe that condition is.

    UNDERSTANDING THE DBQ

    A DBQ is a standardized form created by the Department of Veterans Affairs. It is designed to capture specific medical information about a veteran's health condition in a format that the VA's rating specialists can easily understand and process. There are dozens of different DBQs, each tailored to specific physical or mental health conditions — such as sleep apnea, PTSD, migraines, or irritable bowel syndrome (IBS).

    WHAT INFORMATION DOES A DBQ CAPTURE?

    When a medical provider completes a DBQ, they are evaluating your current symptoms against the criteria the VA uses to assign disability percentages. The form typically requires the provider to document your current diagnosis, the frequency, duration, and severity of your symptoms, any medications you are taking and their side effects, and how the condition impacts your ability to work and perform daily activities (occupational and social impairment).

    DBQ VS. NEXUS LETTER: WHAT'S THE DIFFERENCE?

    Veterans often confuse these two documents, but they serve entirely different purposes in a VA claim. The nexus letter establishes the service connection (answering the question: "Did military service cause or worsen this?"). The DBQ establishes the severity (answering the question: "How bad are the symptoms right now?").

    WHO CAN FILL OUT A DBQ?

    DBQs are typically completed by a VA-contracted doctor during a Compensation and Pension (C&P) exam. However, veterans have the right to submit a DBQ completed by a private, non-VA healthcare provider. Having an independent medical provider complete a DBQ provides an opportunity for your symptoms to be thoroughly documented by a professional who can dedicate time to evaluating your condition comprehensively. As with all medical evidence, the VA will review the submitted DBQ alongside your entire claim file to make a final rating determination.
